Initial Evaluation (60-90 min) | $300-450 Varies based upon time and complexity.
Follow-up visits:
- Standard session (30 min) | $150
- Extended session (50 min) | $225
Emotional Support Animal Evaluation | 30-minutes $125
Payment is due at each scheduled appointment. Regardless of whether you have insurance coverage, all patients are required to keep an active credit card on file. For those patients who will not have insurance coverage, you will receive an itemized invoice (“superbill”) and receipt of payment for all services provided.
The superbill is available for patients to view/download within the patient portal and can be submitted to your insurance company for reimbursement. The superbill/invoice includes the medical clinic tax ID, provider NPI, CPT codes (services provided), and ICD10 codes (diagnoses provided) which should be all the information the insurance needs to submit for reimbursement. It is the patient’s responsibility to handle any desired reimbursement submission. Alternatively, consider using the online service provided by Reimbursify (reimbursify.com) to submit for possible out-of-network insurance reimbursement paid directly to the patient.
I require 48 hours’ notice to cancel an appointment. This allows me the ability to offer your time slot to another patient who may need an appointment. If you cancel your appointment in less than 48 hours or you miss your appointment, a missed appointment fee will be charged. This fee is $150 for a missed Initial Evaluation and $100 for a missed follow-up appointment. These will be charged to your credit card on file.
I do not charge for any administrative time less than 15-minutes. For administrative work that exceeds 15 minutes, I reserve the
right to charge an additional fee of $250/hour in quarter-hour increments. This would include but is not limited to completing paperwork and forms (for schools, employers, etc.), answering emails and messages (rarely takes that long), and prior-authorizations for medications. I will always notify and request permission to proceed prior to engaging in work that would generate an additional fee.
State Insurance
I am either paneled, or in the process of becoming paneled, with most major insurance carriers in the state. These include Aetna, Cigna, Oscar, Oxford, Premera Blue Cross of WA, Regency Blue Shield of WA, and United Healthcare. We will confirm your insurance coverage for you to make sure the care you receive is covered by your insurance plan.
If you do not have insurance, or if your Provider is not paneled with your insurance company, or you choose NOT to use your insurance to pay for your care, you can receive care with our private pay option. See below for current fees.

I do not work directly with insurance companies in these states and bill patients directly at the time of service. I can provide you with an invoice (super-bill) that you may submit to your insurance company, FSA, or HSA plans for reimbursement (which can greatly reduce your costs).
Fee Adjustment Request Form
Practice Commitment: Our telepsychiatry practice is committed to providing accessible mental health care. We allocate 10% of our services either as free care or on a sliding scale fee basis to ensure affordability for individuals with varying financial circumstances. Please note that if we are currently at practice capacity for this level of care, we may not be able to offer free or sliding scale fee services at this time. We will inform you promptly if this is the case.
Free Care: This option is available for individuals who demonstrate significant financial hardship and are unable to afford psychiatric care at standard rates. Our practice evaluates these requests based on household income, number of dependents, and other relevant financial information.
Sliding Scale Fee: Patients eligible for sliding scale fees will pay an adjusted rate based on their household income and financial situation. This allows us to provide care at a reduced cost for those who may not qualify for free care but still require financial assistance.
Confidentiality: All information provided in this form will be kept confidential and used solely for the purpose of evaluating your fee adjustment request. Our practice adheres strictly to patient privacy laws and regulations.
